Coordination - Apply - T-level

The earner of this badge has applied knowledge, skills and understanding of ‘effective coordination’ of health and care services’ in practice. The examples of how they have demonstrated compassion and person-centred care skills, behaviours and knowledge to patients are given in the badge criteria noted below.

Criteria

Candidates that have participated in Health T-level

Badge has been awarded based on attendance of a two year Health T-level course which includes a 315-hour industry placement. Within the placement there was an employer set project which required the individual to apply and contextualise core knowledge through demonstration of core skills.
Worked together for patients’ and have evidence of care and support that is person centred (personalised, coordinated and empowering) - Integrated Care Delivery.
Seen a range of methods of holistic approaches, including how different areas of care have worked together
Seen examples of how the People and Communities 6 principles and enablers
Been involved with multidisciplinary team working and can talk through examples of how team have worked together as part of the organisational structures, including for example how they have taken ownership of their own job roles and responsibilities and how the teams they have worked in have shared relevant information with each other and collaborate to support the continuity of care.
