Coordination - Deepen - T-Level

The earner of the badge has deepened their knowledge and understanding of behaviours for ‘effective coordination’ of health and care services. They have attended three employer led sessions which have focused on the use of technology and importance of 'effective coordination' in day to day activities of job roles within the health and care sector.

Criteria

Candidates that have participated in Health T-level

It has been achieved through attending the Health T-level and gaining knowledge on the importance of 'effective coordination' within the health and care sector, noted below. Learners have also attended three employer led sessions which have focused on use of technology and importance of 'effective coordination' in roles within the sector
The learner has understood that one of the key values of working in the healthcare sector when providing care and support is ‘working together for patients’ and that care and support are person-centred (personalised, coordinated and empowering) - Integrated Care Delivery.
Learner has an understanding of the range of methods of taking a holistic approach to healthcare, which includes integrated working (for example coordinated approach to services through different areas of care, working together with input from the individual.
Learners understand the People and Communities 6 principles, including that services are created in partnership with citizens and communities and that voluntary, community and social enterprise and housing sectors are involved as key partners and enablers (Partnership Principles)
Learner knows how multidisciplinary teams can work together effectively as part of organisational structures and can discuss the importance of different factors in this, including building rapport and positive relationships and the importance of taking ownerships of their own job role.
