Criteria

Candidates that have participated in Health T-level

It has been achieved through attending the Health T-level and gaining knowledge on 'effective coordination' with the health and care sector, noted below. Learners have also attended an employer led session which focuses on use of technology and coordination in roles within the sector.
The learner has understood that one of the key values of working in the healthcare sector when providing care and support is ‘working together for patients’ and that care and support are person-centred (personalised, coordinated and empowering) - Integrated Care Delivery.
Learner has an understanding of the range of methods of taking a holistic approach to healthcare, which includes integrated working (for example coordinated approach to services through different areas of care, working together with input from the individual).
Learners understand the People and Communities 6 principles, including that services are created in partnership with citizens and communities and that voluntary, community and social enterprise and housing sectors are involved as key partners and enablers (Partnership Principles)
